STRATEGIC, TACTICAL AND OPERATIONAL ASPECTS OF DEVELOPMENT OCCUPATIONAL MEDICINE IN RUSSIA

Abstract: The article presents a comprehensive analysis of the current state of the occupational medicine system in the Russian Federation and develops a multilevel model of its development. Based on a systematic approach, key problems have been identified: fragmentation of regulatory and legal regulation, the predominance of formal control over prevention, staff shortages, digital lag, and weak integration of workplace health promotion programs. Special attention is paid to the macroeconomic consequences: The total losses from unsatisfactory working conditions are estimated at 4% of GDP, and the annual losses of the working time fund reach 92.5 million man-days. A three-level model is proposed as a methodological basis for the solution, including strategic (development of a national strategy, harmonization of legislation, creation of a digital platform), tactical (development of human resources, implementation of risk-oriented models, pilot projects with expert systems) and operational measures (audit of local acts, trainings, daily monitoring).The central result of the research is the development of a multifactorial economic and mathematical model, quantifying investments in improving working conditions with reducing the loss of working time. The practical testing of the model on a hypothetical production case demonstrates an annual economic efficiency of 11.7 million rubles. with an investment of 2.5 million rubles, which confirms the high profitability of preventive measures. The practical significance of the work lies in the formation of specific, economically sound recommendations for government authorities, employers and the medical community aimed at reducing occupational risks, preserving the nation’s employable potential and minimizing significant economic damage.
